Anesthetic management of achondroplastic parturient undergoing cesarean section

Abstract
Achondroplasia is a disease of dwarfism due to underdevelopment of the bone and premature ossification. A cervical spine instability, deformation of the thoraco-lumbar spine, large tongue and mandible are the usual manifestation of the achondroplasia. These clinical problems may lead to technical failure in regional anesthesia or airway problems in general anesthesia. In the state of pregnancy, these ¡¯dwarves¡¯ may suffer from low functional residual capacity, tracheal edema, spinal compression of the fetus, large breasts and increased chance of aspiration pneumonia that require delicate anesthetic care. Regional anesthesia is not only less preferred in these patients for technical reasons and danger of cord injury but also lacks definite criteria about the drug dose. In this study we report a case of caesarean section in a 29-year-old achondroplasia patient, who did not suffer any technical, neurological and hemodynamical problems from induction to the recovery. We would like to discuss about the regional anesthesia in these patients.
